The following Privacy Policy (hereinafter referred to as "this Policy") outlines how we handle users' personal information.

Article 1 (Personal information)

"Our hospital may ask users for personal information such as their name, date of birth, address, telephone number, email address, bank account number, credit card number, and driver's license number when they register to use our services. We may also collect transaction records and payment information containing users' personal information from our partners (including information providers, advertisers, and advertising distributors; hereinafter referred to as "partners").

Article 3 (Purpose of collecting and using personal information)

The purposes for which our hospital collects and uses personal information are as follows:
  • 1. For the provision and operation of our hospital services.
  • 2. To respond to inquiries from users (including verifying their identity)
  • 3. To send emails to users regarding new features, updates, campaigns, etc., of the services they are using, as well as information about other services provided by our hospital.
  • 4. For maintenance, important announcements, and other necessary communications.

Article 4 (Changes to the purpose of use)

  • 1. Our hospital will only change the purpose of use of personal information if it is reasonably deemed to be related to the original purpose of use.
  • 2. If the purpose of use is changed, the hospital will notify users of the changed purpose by a method prescribed by the hospital or publish it on this website.

Article 5 (Provision of personal information to third parties)

  • 1. Our hospital will not provide personal information to third parties without obtaining the user's prior consent, except in the following cases. However, this excludes cases permitted by the Personal Information Protection Act and other laws and regulations.
    • a. When it is necessary to protect a person's life, body, or property, and it is difficult to obtain the person's consent.
    • b. When it is particularly necessary for the improvement of public health or the promotion of the sound upbringing of children, and it is difficult to obtain the person's consent.
    • c. When it is necessary to cooperate with a national or local government agency or an entity entrusted by them in carrying out duties prescribed by law, and obtaining the person's consent would likely hinder the performance of those duties.
    • d. When the following matters have been notified or publicly announced in advance, and the hospital has notified the Personal Information Protection Committee.
      • i. The purpose of use includes providing the data to a third party.
      • ii. Items of data provided to third parties
      • iii. Means or methods of providing to third parties
      • iv. To cease providing personal information to third parties upon the individual's request.
      • v. Method of accepting requests from the individual
  • 2. Notwithstanding the provisions of the preceding paragraph, in the following cases, the recipient of such information shall not be considered a third party:
    • a. When our hospital entrusts all or part of the handling of personal information to a third party to the extent necessary to achieve the purpose of use.
    • b. When personal information is provided in connection with the succession of a business due to merger or other reasons.
    • c. When personal information is jointly used with a specific party, and the party has been notified in advance, or the information has been made easily accessible to the party, regarding the fact of joint use, the items of personal information to be jointly used, the scope of the parties jointly using the information, the purpose of use by the parties, and the name or title of the person responsible for the management of the personal information.

Article 6 (Disclosure of Personal Information)

  • 1. When an individual requests disclosure of their personal information, our hospital will disclose it to the individual without delay. However, if disclosure would fall under any of the following categories, we may choose not to disclose all or part of the information, and if we decide not to disclose, we will notify the individual of that decision without delay. Please note that a fee of 1,000 yen per request will be charged for the disclosure of personal information.
    • a. When there is a risk of harming the life, body, property, or other rights and interests of the person or a third party.
    • b. When there is a risk of significantly hindering the proper conduct of the hospital's operations.
    • c. Any other case that would violate laws and regulations
  • 2. Notwithstanding the provisions of the preceding paragraph, information other than personal information, such as historical information and characteristic information, will not be disclosed in principle.

Article 7 (Correction and Deletion of Personal Information)

  • 1. If a user believes that their personal information held by our hospital is inaccurate, they may request our hospital to correct, add to, or delete (hereinafter referred to as "correction, etc.") their personal information in accordance with the procedures established by our hospital.
  • 2. If our hospital receives a request from a user as described in the preceding paragraph and determines that it is necessary to comply with the request, we will correct the personal information in question without delay.
  • 3. If our hospital makes corrections or other changes based on the provisions of the preceding paragraph, or if we decide not to make any corrections or other changes, we will notify the user without delay.

Article 8 (Suspension of Use of Personal Information, etc.)

  • 1. Our hospital may, upon request from the individual, cease using or delete their personal information on the grounds that it is being handled beyond the scope of the intended purpose of use, or that it was obtained through fraudulent means.
  • 2. Based on the results of the investigation in the preceding paragraph, if it is determined that it is necessary to comply with the request, the use of the personal information in question will be stopped without delay.
  • 3. If our hospital suspends or terminates the use of the service pursuant to the provisions of the preceding paragraph, or if we decide not to suspend or terminate the use of the service, we will notify the user without delay.
  • 4. Notwithstanding the preceding two paragraphs, if suspending or discontinuing service would incur significant costs or is otherwise difficult, and if alternative measures necessary to protect the user's rights and interests can be taken, such alternative measures will be implemented.

Article 9 (Changes to the Privacy Policy)

  • 1. The contents of this policy may be changed without notice to the user, except for matters stipulated otherwise in this policy or by law.
  • 2. Unless otherwise specified by our hospital, the revised privacy policy will take effect from the time it is posted on this website.
